Job summary

Heartbeat Health is seeking a Nurse Practitioner to join our virtual cardiology clinic. You will deliver patient-centered care using telemedicine, focus on Heart Failure management, GDMT titration, and collaboration with cardiologists and PCPs.

Ideal candidates have an MSN with cardiology specialization and 3+ years in outpatient cardiology, with NP and cardiology certifications. This remote role follows CST hours, Monday–Friday.

Qualifications

• MSN with cardiology specialization.
• 3+ years as an outpatient cardiology NP.
• Active NP license (prefer Compact State).
• NP certification in cardiology (ANCC/AANPCB).
• ACLS certification recommended.
• Proficiency in interpreting ECGs and echocardiograms.
• GDMT mastery and titration.
• Experience delivering care via telehealth.

Responsibilities

• Assess patients via telehealth, develop treatment plans.
• Prescribe meds and order diagnostic tests.
• Manage heart failure and GDMT remotely.
• Collaborate with cardiologists and PCPs.
• Educate patients and families on care plans.
• Maintain accurate clinical documentation.
• Engage in quality improvement and research.
