Job summary

Movn Health is seeking care managers to join our team delivering digital cardiac rehabilitation to patients. You will guide participants via 1:1 video calls, develop personalized treatment plans, and coordinate with Medical Directors, with weekly coaching and remote monitoring.

Qualifications

Bachelor degree in exercise related science or nursing.
1–2 years of telehealth, remote patient monitoring or chronic care management experience.
Certification: ACSM-CEP or CCRP, or RN with CCRP (or willing to obtain).
Competent with digital collaboration tools and videoconferencing.

Responsibilities

Guide participants in 1:1 video calls and group education sessions in a 3 month online program.
Develop comprehensive treatment plans tailored to individual risk factors.
Provide coaching, education, and support via video, voice or text messages weekly.
Coordinate approval of treatment plans with Medical Directors remotely.
Share progress reports with referring providers monthly.
