Job summary

Carda Health is transforming cardiac rehab through remote, telehealth-enabled care in partnership with leading health systems. You will lead emotionally safe group sessions, guide patients in a transdisciplinary program, and support our digital care delivery platform.

We seek a licensed mental health professional with strong group facilitation skills, experience in behavioral health or cardiopulmonary care, and comfort troubleshooting devices and wearables in a fast-growing startup.

Qualifications

• Minimum 2+ years of clinical or facilitation experience.
• Licensed clinical social worker or licensed professional counselor.
• Strong proficiency with mobile technology and telehealth platforms.
• Excellent interpersonal and organizational skills.

Responsibilities

• Lead group support sessions for cardiac rehab patients according to clinical protocols.
• Deliver high quality care for patients in rehab programs and clinical studies.
• Wear many hats, including education during Lifestyle Classes and tech support.
• Collaborate with founders to improve digital care delivery models.
• Provide support to patients, promoting accountability and engagement.
• Work with the technology team to ensure a smooth patient experience.
