Job summary

Movn Health is seeking care managers to join our team and mission to provide digital cardiac rehabilitation to 1 million patients annually. In this role you will manage patients weekly via telehealth to improve risk factors and reduce future cardiac events, with flexible hours and a path toward potential full-time employment.

Qualifications

• Bachelor degree in exercise related science or nursing.
• 1-2 years telehealth/remote care experience.
• Experience in cardiac education, care management or rehab is a plus.

Responsibilities

• Guide participants in 1:1 video calls and group education sessions to manage cardiac risk.
• Develop tailored treatment plans addressing exercise, meds and lifestyle.
• Provide weekly coaching, support and adjust plans as needed.
• Coordinate treatment plan approvals with Medical Directors remotely.
• Share progress with referring providers monthly and monitor for adverse events.
