Job summary

Aviary Health is seeking a Clinical Eligibility & Intake Nurse for our virtual cardiac rehab program. This Part Time/1099 Contractor role is fully remote in the United States, with 15-20 hours per week and a Monday–Friday schedule that overlaps intake hours.

The nurse will review EMR records to determine eligibility and, when needed, perform initial video assessments with new patients. Strong EMR and documentation skills are essential.

Qualifications

• Active RN or LPN license required; multistate/compact license preferred.
• 3–5 years of clinical experience in cardiac care, rehab, or chronic disease management.
• 3 years of experience in Epic or another EMR, including chart review.
• Fluent in English (written and verbal).
• Proficient with Zoom, Google Suite and EMR systems.

Responsibilities

• Review patient charts in Epic and other EMRs to determine medical eligibility.
• Identify current cardiac rehab referral orders, or create orders when needed.
• Communicate eligibility decisions to the enrollment team with concise documentation.
• Work the eligibility queue within team turnaround targets.
• Conduct initial video assessments for intake coverage when needed.
• Educate patients on program benefits and risk factor modification.
