Aviary is seeking a Clinical Eligibility & Intake Nurse to work remotely on a part-time, 1099 contract basis. You will review EMR charts to determine eligibility for our virtual cardiac rehab program and step in for intake assessments as needed.

The role blends chart review with direct patient interaction via video, requiring strong clinical judgment and clear documentation. The ideal candidate holds an active RN or LPN license, has 3–5 years in cardiac care or rehab, and is proficient with

Qualifications

Active RN or LPN license required, multi-state/compact preferred.
3-5 years in cardiac care, rehab, or chronic disease management.
3 years with Epic or another EMR, including chart review.
Fluent in English; bilingual in English and Spanish a plus.
Experience with virtual care or 1099 contract work is beneficial.

Responsabilités

Review EMR charts to determine medical eligibility for a virtual cardiac rehab program.
Identify eligibility orders and document decisions clearly for enrollment.
Perform intake assessments by video when needed and develop treatment plans.
Educate patients on program benefits and risk-factor modification.
Document visits accurately and support team meetings with documentation.
