Job summary

Aviary Health is seeking a Cardiac Rehab Intake Specialist to support our remote cardiac recovery program. You will partner with exercise physiologists, patient experience, product, and marketing teams to assist patients via video calls, track progress, and provide feedback.

Experience with cardiac conditions, CHF, and related areas is preferred. The role involves initial assessments, education on program benefits, and guiding patients through remote care, with weekend shift options and a 1099

Qualifications

• Minimum of three years helping people manage their health, ideally in a clinical or remote health coaching setting.
• Fluent in English (written and verbal).
• Bilingual in English and Spanish is a huge plus.

Responsibilities

• Conducting initial assessments for patients enrolling in the virtual cardiac recovery program.
• Educating patients on program benefits and risk-factor modification.
• Coaching individuals through electronic messaging and video sessions.
• Adapt in-clinic best practices to a remote-care delivery model.
• Complete individualized treatment plans including nutrition, psychosocial, fitness, and co-morbidity assessments.
