Job summary

Angels of Care Pediatric Home Health is seeking an RN Nurse Navigator to support Star and Star Kids clients remotely from Texas. The role coordinates care across the continuum, working with clinicians to manage Medicaid services and private duty nursing services for pediatric patients.

You will collaborate with care teams, coordinate hospital referrals, schedule follow-ups, document in EMR, and advocate for families.

Qualifications

• RN license required.
• Bilingual in Spanish and English.
• 2+ years of case management experience in home health, managed care, or hospital case management (pediatric experience preferred).
• 2+ years of clinical experience in pediatric ambulatory care, inpatient, or home care.

Responsibilities

• Collaborate with patients, families, physicians and staff to ensure high quality care for PDN clients.
• Coordinate with client’s care team to improve service coordination and lessen family burden.
• Coordinate hospital admissions and discharge planning to ensure referrals are in place.
• Schedule follow-up physician appointments and conduct routine check-ins.
