• Strong communication and organizational skills.
• Ability to multitask and work under pressure while maintaining member experience.
• Know when to escalate in emergent situations.
• Strong technical skills to navigate documentation systems and learn new ones.
• Ability to drive during day, night, or inclement weather.
• Valid driver's license with safe driving record.
• State-required automobile insurance.
• Comfort with rapid changes to business strategy and priorities.

Responsibilities

• Administer multiple assessments and screening tools at various times in the care timeline.
• Interpret assessment results and prioritize needs.
• Be accountable for care plan development within the multidisciplinary team.
