Job summary

Angels of Care Pediatric Home Health is seeking an RN Nurse Navigator to join the Angel Link Health Home Services team, supporting Star and Star Kids clients. This remote role coordinates care across the continuum and manages Medicaid resources to improve family outcomes.

Based in Texas, you will collaborate with physicians, hospitals, and care teams; maintain detailed EMR records; educate families; and advocate for patients while balancing clinical and service coordination duties.

Qualifications

• RN required.
• Bilingual in Spanish and English required.
• 2+ years Case Management experience in home health, managed care, or hospital case management (pediatric case management experience preferred).
• 2+ years of clinical experience in pediatric ambulatory care, inpatient setting, or home care.

Responsibilities

• Collaborate with patients, families, physicians and staff to ensure high quality care for PDN clients.
• Coordinate with hospital and MCOs during admissions and discharge planning to ensure referrals are in place.
• Schedule follow-up physician appointments and provide routine check-ins to ensure attendance.
• Communicate client needs with care teams and advocate for patient needs.
• Maintain comprehensive clinical records in the EMR and Health Home Hub.
• Develop trust and build rapport with clients and families.
