Qualifications

• Active RN license in New Jersey (unrestricted).
• Bilingual in Spanish and English (written and verbal).
• Minimum 2 years clinical nursing experience.
• Experience with EMR/EHR systems.

Responsibilities

• Conduct patient outreach and follow-up in Spanish and English.
• Provide nursing support, patient education, and care coordination.
• Document patient interactions accurately in EMR/EHR systems.
• Explain medications and health resources to patients.
• Support case management and clinical support activities as needed.
