Legacy Community Health is seeking a Bilingual PrEP Health Navigator to provide virtual patient navigation, education, and support services that emphasize HIV prevention and PrEP care. The role emphasizes remote engagement with patients and community members to improve access and adherence.

Qualifications

• Background in HIV prevention or health navigation.
• Experience educating diverse populations, one-on-one or in groups.
• Experience with electronic health records systems (EHRS).
• Bilingual English/Spanish preferred.

Responsibilities

• Conduct virtual outreach to identify individuals who may benefit from PrEP.
• Provide remote patient navigation and schedule appointments with clinical teams.
• Deliver health education on PrEP, HIV prevention, and safer sex practices.
• Support retention in PrEP care with follow-up and adherence support.
• Maintain updated documentation in the EHR and coordinate with care teams.
