Job summary

Community Health Center, Inc. is seeking a Prevention Health Navigator for the Center for Key Populations in Middletown, CT.

This role focuses on increasing PrEP awareness, facilitating access, and linking clients to care, with emphasis on reducing barriers and providing navigation and support. The navigator will educate communities, assist with health insurance enrollment, appointment scheduling, and financial assistance where needed, and conduct readiness screenings to promote ongoing PrEP

Qualifications

• Associate degree in healthcare, public health, social work, or related field is required.
• Bilingual Spanish and English proficiency is required.
• Experience in healthcare settings, community outreach, or navigation of community resources preferred.
• Proficiency with Microsoft Office and familiarity with EHR systems is preferred.

Responsibilities

• Increase awareness of PrEP and provide direct linkage to care for individuals seeking PrEP services.
• Offer navigation support for health insurance enrollment, PrEP provider identification, and appointment scheduling.
• Conduct readiness screenings for PrEP and assess barriers to care and available financial assistance.
• Maintain accurate patient records in electronic health systems and ensure confidentiality.
