Job summary

Medix seeks a bilingual Spanish/English LPN/LVN to join a remote healthcare team supporting members through telephonic medication management and outreach. This role suits an LPN who communicates by phone, works independently, and thrives in a high-volume, metrics-driven environment.

You will support medication adherence, identify medication-related concerns, educate members, and connect them with the appropriate clinical resources.

Qualifications

• Bilingual Spanish/English communication required.
• Active LPN/LVN license in good standing.
• Graduate of an accredited LPN/LVN program.
• Experience in healthcare and/or telephonic outreach.
• Ability to navigate electronic documentation systems and Microsoft Office.

Responsibilities

• Conduct telephonic medication interviews for MTM.
• Verify member identity and obtain consent before discussing meds.
• Review and reconcile prescription, OTC, herbals, and supplements.
• Assess adherence, effectiveness, side effects, affordability, barriers.
• Document interactions, assessments and outcomes in systems.
• Provide education on administration, dosing, storage, and adherence.
