Job summary

PharmD Live is seeking a bilingual Spanish-speaking Licensed Practical Nurse (LPN) to support Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) programs. This role focuses on improving health outcomes for patients with chronic conditions by delivering patient-centered care, education, and support in both English and Spanish.

The position is fully remote with flexible hours. Colorado LPN licensure is required, and candidates should have at least 2 years of relevant experience,

Qualifications

• Active Colorado LPN license in good standing.
• Bilingual in Spanish and English (oral and written).
• At least 2 years in patient care or care coordination.

Responsibilities

• Conduct comprehensive assessments of patients' health needs.
• Collaborate with care teams to develop care plans.
• Educate patients in Spanish and English on disease management and self-care.
• Coordinate care, referrals, and test results across providers.
• Monitor RPM data and elevate concerns as needed.
• Document interactions in the EHR per CMS CCM/RPM guidelines.
• Support enrollment into CCM and RPM programs.
