Concert Health in the United States is hiring a NY Licensed Bilingual Spanish/English Clinician (LMSW or LCSW) for a remote position. This professional will provide Collaborative Care under supervision, support primary care integration, and deliver evidence-based interventions to pediatric and adult patients with mild to moderate symptoms.

The role emphasizes multilingual clinical documentation, care coordination with PCPs, and supervision opportunities.

Qualifications

• Master’s degree in social work, counseling, or related field from an accredited program.
• Licensed to practice in New York as a clinician (LMSW, LCSW or equivalent).
• Proficient in English and Spanish communication per industry standards.

Responsabilités

• Provides care to patients based on assigned Care Pod, tailoring treatment plans to individual needs.
• Coordinates behavioral health care with the patient’s primary care provider and other services.
• Screens patients for behavioral health disorders using PHQ9, GAD7, and other tools.
• Monitors symptoms and treatment side effects to reduce symptoms.
• Supports psychotropic medication management with PCP oversight and adherence.
