TailorCare, Inc. is hiring a Bilingual Clinical Navigator - PT to deliver high-quality clinical navigation via phone and video in a fast-paced, metrics-driven environment.

You will engage members, assess needs, build trust, drive participation, and support adherence to evidence-based care plans. The role requires a DPT/MPT and an active state license across GA/TX/CO/IN (compact preferred), with up to 10% travel.

Qualifications

• DPT or MPT degree required.
• Current unrestricted Physical Therapy license in GA, TX, CO and/or IN; compact license preferred.
• Willingness to obtain additional state licenses as TailorCare expands.
• Minimum 3 years of experience in PT, musculoskeletal care, care navigation, telehealth, or related clinical settings.
• Proven success managing patient interactions in high-volume, performance-oriented environments.
• Strong communication, motivational interviewing, and patient engagement skills.
• Experience using technology and data to improve workflow and outcomes.
• Ability to work independently with productivity, engagement, and quality targets.
• Comfort with multiple technology platforms, dual-monitor setups, and telehealth tools.
• Willingness to travel up to 10% for onsite meetings and events.

Responsabilités

• Serve as the primary navigator for an assigned member population in a contact center environment.
• Conduct comprehensive clinical intake assessments for MSK conditions and goals.
• Deliver navigation via telephonic and video interactions; select the best modality.
• Maintain high volume of member interactions with a personalized, evidence-based approach.
• Build trust using motivational interviewing and behavioral coaching.
• Educate members on treatment options and resources for informed decisions.
• Proactively follow up to drive adherence and engagement.
• Monitor progress and identify needs for escalation or support.
• Advocate for members to ensure continuity of care throughout the journey.
• Contribute to development and delivery of virtual PT programs and digital MSK care.
