Job summary

Valenz Health is seeking a Bilingual Case Management Nurse to manage comprehensive, patient-centered care remotely. You will assess needs, develop care plans, and coordinate with multidisciplinary teams to ensure continuity of care across diverse health needs.

In this fully remote role, you will support patients navigating the healthcare system, educate on medications, promote adherence, and contribute to wellness initiatives while ensuring HIPAA compliance.

Qualifications

• Active RN license in home state or state of residence.
• Bilingual in English and Spanish (oral and written).
• 3+ years of nursing experience in an acute care setting.

Responsibilities

• Coordinate and manage healthcare services for comprehensive, patient-centered care.
• Assess patients’ medical needs and develop individualized care plans.
• Advocate for patients’ rights, preferences, and access to services.
• Monitor progress, adherence to care plans, and adjust interventions as needed.
• Document assessments, plans, and outcomes accurately and promptly.
• Educate patients on medications, treatment plans, and self-care practices.
