Job summary

PeopleOne Health is seeking a Behavioral Health Navigator to integrate with clinicians, social workers, and external partners in Florida. You will screen, coordinate care for therapy and mental health services, and provide SDOH screenings and referrals to improve member well-being.

Work focuses on collaboration within a multidisciplinary team, community outreach, and maintaining accurate EHR documentation while supporting crisis situations and ongoing member education.

Qualifications

• Bachelor’s degree in social work, psychology, counseling, or related field.
• Or CMA certification with 3–5 years in behavioral health or care coordination.
• Experience with behavioral health, care navigation, or community outreach.

Responsibilities

• Support the social worker in addressing mental, behavioral, and social needs.
• Coordinate care for therapy, psychiatry, and community resources.
• Provide SDOH screenings, resources, and referrals.
• Collaborate with multidisciplinary care team to align health needs.
• Conduct community outreach and build partnerships with local organizations.
• Coordinate with local substance use treatment providers for assessments and referrals.
• Provide behavioral health education for members and employees.
• Document interactions and outcomes in the EHR.
