Job summary

PeopleOne Health is seeking a Behavioral Health Navigator to join our care team. You will work with clinicians, social workers, and external partners to address mental, behavioral, and social needs, focusing on screening, care coordination, and resource navigation to improve member well-being.

The role emphasizes collaboration with a multidisciplinary team, community outreach, and crisis support while maintaining HIPAA-compliant documentation in the EHR.

Qualifications

• Bachelor’s degree in social work, psychology, counseling, or related field.
• CMA with 3–5 years in behavioral health, primary care, care coordination, or patient navigation.

Responsibilities

• Support social worker in addressing mental, behavioral, and social needs within the area.
• Coordinate care for therapy, psychiatry, and mental health resources with external services.
• Provide SDOH screenings, resources, and referrals.
• Collaborate with multidisciplinary care team to align physical and behavioral health needs.
• Conduct community outreach and build partnerships with local organizations.
• Coordinate with local substance use treatment providers for assessments and referrals.
• Provide behavioral health education for members and employees.
• Track completion of mental health screenings and documentation in the EHR.
• Participate in team huddles, care conferences, and staff meetings.
