Job summary

Concert Health is a national behavioral health medical group enabling remote Collaborative Care integration. This role supports evidence-based interventions, symptom monitoring, and outcomes tracking across pediatric and adult populations, reporting to the Clinical Excellence Manager and collaborating with the care team.

Qualifications

• Screen patients for common behavioral health and substance use disorders using PHQ9, GAD7, and Columbia Suicide Risk Assessment.
• Monitor patients for changes in symptoms and treatment side effects, and support medication management prescribed by PCPs.
• Track patient outcomes using a registry and document progress in Salesforce and the electronic health record.

Responsibilities

• Provide evidence-based interventions via Collaborative Care Model.
• Work with pediatric and/or adult populations to promote symptom reduction.
• Report to the Clinical Excellence Manager and collaborate with a care team.
