Job summary

Blue Cross and Blue Shield of Minnesota seeks a Behavioral Health Case Manager to provide telephonic support, assess needs, and develop individualized care plans for members. You will coordinate referrals, connect members to providers, and help them understand benefits while avoiding direct clinical care.

You’ll handle a mix of new and follow-up calls, document findings, and collaborate with multidisciplinary teams to ensure timely, person-centered care while respecting privacy requirements.

Qualifications

• 3+ years of related case management (clinical) experience.
• Bachelor's degree; in lieu of a degree, two years of relevant experience may be accepted.

Responsibilities

• Provide telephonic engagement using motivational interviewing and therapeutic techniques to encourage participation.
• Develop and monitor individualized care plans with follow-up actions and goals.
• Navigate multiple systems to gather information and update case documentation.
• Review member information and prepare for outreach calls to conduct assessments and document findings.
• Coordinate referrals and connect members to community and in-network providers.
