Job summary

Comagine Health is seeking a professional to provide statewide eligibility determination and service coordination for individuals with serious persistent mental illness. This position requires strong clinical skills and the ability to conduct face-to-face assessments and develop treatment plans. Ideal candidates will have a background as a Qualified Mental Health Professional (QMHP) or similar qualifications and be located in or near Eugene, Oregon.

Qualifications

• 2+ years of clinical experience with assessments for SPMI.
• Must be a Qualified Mental Health Professional (QMHP) or have unrestricted clinical licensure in Oregon.
• Willingness to travel and conduct in-person assessments.

Responsibilities

• Administer face-to-face assessments and service planning.
• Monitor service plans continuously.
• Assist with transitions between community and institutional settings.
