Job summary

Comagine Health is seeking a clinician to conduct assessments and develop treatment plans for individuals with serious mental illness. The role supports statewide initiatives and involves substantial travel to perform in-person assessments.

The ideal candidate holds QMHP eligibility or Oregon licensure, with 2+ years of clinical experience, and resides in the Eugene, Oregon area, as travel up to 100% may be required.

Qualifications

• 2+ years clinical experience including assessments, histories, examinations, and DSM-5 diagnoses.
• QMHP eligibility or Oregon licensure required or to be obtained within 2 weeks of offer.
• Residence in the geographic area with the ability to travel up to 100% for in-person assessments.

Responsibilities

• Conduct face-to-face assessments and person-centered service planning with individuals.
• Monitor service plans and coordinate transitions between community and institutional settings.
• Collaborate with Oregon Health Authority partners to support eligibility determination and planning.
