Behavioral Health Case Manager II at Elevance Health

The Behavioral Health Case Manager II will work with our behavioral health members diagnosed with mental illness disturbances. The role requires meeting with members in person as per contractual requirements. The candidate will be responsible for performing case management telephonically and/or by home visits within the scope of licensure for members with behavioral health and substance abuse or substance abuse disorder needs. The subject matter expert will focus on targeted clinical areas such as Eating Disorders, Maternity Alcohol/Drug, Autism Spectrum Disorders, and other disorders.

• Supports member access to appropriate quality and cost effective care.


• Coordinates with internal and external resources to meet identified needs of the members and collaborates with providers.


• Serves as a resource to other BH Case Managers.


• Participates in cross‑functional teams projects and initiatives.
