Job summary

Elliot Health System in Manchester, NH is seeking a Behavioral Health Case Manager who will provide clinical social work with pediatrics, adolescents, adults, and geriatrics. The role includes assessing treatment needs, developing and monitoring plans, and coordinating care across the continuum.

The BHCM will work remotely with the EBHS team, ensuring effective communication, transition planning to community services, and adherence to professional ethics, with a focus on client-centered outcomes.

Qualifications

• Master's degree in social work, Mental Health Counseling or related field
• Experience in social service setting with working knowledge of case management systems and planning techniques
• Strong communication and coordination skills with diverse populations

Responsibilities

• Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-focused
• Provide ongoing support and case management expertise to clients' needs
• Educate and support transition of patients to community-based services
• Ensure all client case management plans are current and address identified needs
• Communicate effectively with individuals from diverse cultures and backgrounds with professional diplomacy
• Monitor and evaluate client progression and update plans as needed
• Maintain documentation of service objectives and outcomes per guidelines
• Provide information, referrals, and facilitate linkages to mental health treatment and housing programs
• Participate in client-related meetings and trainings
• Administer screenings for mental health and substance use disorders
• Maintain client data systems and EPIC entries
• Prepare case management reports on outcomes and successes
• Adhere to boundaries and professional ethics
• Assess crisis situations and respond/refer appropriately
