The position emphasizes collaboration with families, schools, and healthcare providers, maintaining documentation, and using EPIC. Remote work is available with in-office days on M/W/F and virtual Tue/Thu.

Qualifications

• Master’s degree in social work or related field.
• Experience in social services with case management.
• Knowledge of case management systems and planning techniques.
• Ability to work in a primarily remote BHCM role.

Responsibilities

• Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-focused.
• Provide ongoing support and case management expertise to clients' needs.
• Educate and support transition of patients to community-based services.
• Ensure that all client case management plans are current and address identified needs.
• Communicate with diverse cultures and backgrounds and act with professionalism.
• Monitor progress and revise plans as needed; maintain documentation per guidelines.
• Arrange referrals and linkages to mental health services, housing, and public programs.
• Participate in client meetings and trainings; administer screenings.
• Maintain EPIC entries and prepare case-management reports.
