Job summary

Elliot Health System in Manchester, NH seeks a Social Worker Case Manager in Geropsychiatric care to support adults with psychiatric conditions across the care continuum. Responsibilities include clinical assessment, discharge planning, and coordination with the care team to maximize safety and recovery.

You will provide counseling, crisis intervention, and family education, facilitate unit rounds, and advocate for patient needs while maintaining confidentiality and ethical standards.

Qualifications

• Master's degree in Social Work required.
• NH LICSW preferred.
• 1–3 years in a healthcare/hospital setting preferred.
• Excellent clinical documentation and EMR experience.

Responsibilities

• Patient Facilitation: collaborate with the nursing team on psychosocial assessment and discharge planning.
• Utilization Management/Managed Care: address delays and assess continued need for acute care.
• Quality Management: participate in process improvement and data collection.
• Discharge Planning: develop comprehensive plans for complex patients and coordinate community resources.
• Other: assist with behavioral health units and related duties.
