Job summary

Centene Corporation is seeking a licensed behavioral health professional to develop and coordinate holistic care for members with significant mental health needs in a hybrid Tyler, TX role. Responsibilities include home visits, education on services, and care plan development with coordination across providers.

The position requires a master’s degree in Mental Health Counseling or Social Work and state licensure (LCSW/LMFT/LMHC/LPC) with 2–4 years of related experience.

Qualifications

• Requires a master's degree in Mental Health Counseling or Social Work and 2–4 years of related experience.
• Unrestricted behavioral health professional license based on state contract requirements (e.g., LCSW, LMFT, LMHC, or LPC) is required and must be licensed in the applicable state.
• Experience collaborating with medical and behavioral health providers and managing community resources for Foster Care Medicaid recipients is a plus.

Responsibilities

• Evaluates the needs of the most complex high‑risk members with mental/behavioral health needs and develops a plan of care.
• Acts as liaison and member advocate between member/family, physician, and facilities.
• Performs frequent home visits to assess needs and collaborate with resources.
• Educates members and families on preventive care, treatment options, and benefits.
• Provides coordination of service authorization for various services (meals, housing, transportation, etc.).
• Maintains documentation in the electronic care management system and ensures compliance.
