Job summary

Humana Inc. is seeking a qualified Care Manager, Telephonic Behavioral Health 2 to assess member needs on a remote basis and coordinate care with families.

You will help members navigate resources and monitor wellness outcomes, occasionally creating care plans and resolving barriers. The role requires a valid behavioral health license (LMSW/LCSW/LPC/LMHC), a Master’s in a behavioral-health field, and at least 2 years of experience; proficiency with Microsoft Office and telephonic care is

Qualifications

Current valid social service license (LMSW/LCSW/LPC/LMHC) required.
Master's degree in a behavioral-health field from an accredited university.
2+ years of experience as a behavioral health professional.
Experience with behavioral change, health promotion, and coaching.
Proficiency with Microsoft Office programs (Word, Excel, PowerPoint).

Responsibilities

Assess and evaluate members' needs to achieve wellness goals.
Guide members/families toward appropriate resources and care interactions.
Identify and resolve barriers to effective care and monitor progress.
Create member care plans when needed and follow guidelines/procedures.
Operate in telephonic environment with independent decision making.
