Job summary

Molina Healthcare seeks a Texas-licensed Behavioral Health Professional to support Medicaid members with behavioral health needs via telephonic care coordination, with occasional in-person visits. Qualifications

• At least 2 years health care experience, preferably in behavioral health.
• Licensed behavioral health clinician with active license in state practice.
• Valid driver's license and reliable transportation for travel.
• Experience with severe mental health conditions and substance use disorders.
• Knowledge of whole person care principles and discharge planning.
• Data entry skills and experience using a clinical platform.
• Excellent verbal and written communication skills.
• Microsoft Office suite proficiency.

Responsibilities

• Completes comprehensive behavioral health assessments per timelines to identify candidates for care coordination.
• Develops and implements care plans with members, families, and healthcare professionals.
• Conducts telephonic, in-person, or home visits as required.
• Monitors care plan effectiveness and documents interventions and goal achievement.
• Maintains ongoing member caseload for regular outreach.
• Promotes integration of behavioral health, LTSS, and community resources.
• Facilitates interdisciplinary care team meetings and ICT collaboration.
• Uses motivational interviewing and Molina guidelines to educate and support change.
• Assesses barriers to care and provides coordination to address concerns.
• May provide consultation and resources to peers as needed.
• 25-40% local travel may be required.
