• Manage provider panels and waitlist to maximize access.
• Support high-risk lists and outreach per provider direction.
• Track outcomes and escalate access issues through channels.
• Collaborate with operations, providers, leadership and cross-functional partners.
• Perform additional duties as assigned by Supervisor/Manager.
