Job summary

SpringHealth Behavioral Health and Integrated Care is seeking a clinician experienced in behavioral interventions to assess patients, coordinate care, and support families in multiple settings across the community.

The role involves scheduling, home visits, and collaboration with an interdisciplinary team to ensure effective implementation of intervention plans and accurate documentation.

Qualifications

• Experience with behavioral intervention or educational emphasis on learning and developmental psychology.
• 2+ years of experience with individuals with developmental disabilities or behaviorally challenged population.
• Reliable transportation.
• Ability to perform physical tasks including standing for extended periods, bending, and moving between seated and standing.
• State-specific requirements vary by state (BCBA/BCaBA, licensed psychologist, or related qualifications).

Responsibilities

• Conducts ongoing assessments to meet patients' needs.
• Communicates between supervisors, direct care staff, family members, guardians, and collateral individuals regarding progress.
• Provides a monthly schedule of appointments, home visits, and observations to monitor progress.
• Coordinates home, work, and community-based visits to meet observation and data analysis requirements.
• Ensures the Behavior Intervention Plan is implemented effectively.
• Participates in Interdisciplinary Team Meetings and other pertinent meetings.
• Completes monthly and/or quarterly progress notes.
• Organizes training to certify staff in implementing the Behavior Intervention Plan and documentation sheets.
• Provides follow-up interviews, home visits, and observations for continued support.
• Maintains strong clinical records for easy access by interested parties.
• Acts as liaison with public and mental health communities to support ongoing goals.
• Assists in creating procedures for training materials to improve programs.
