Job summary

Shields Health Solutions seeks an Ambulatory Clinical Pharmacist to deliver diabetes and obesity-focused medication therapy management. The role emphasizes safe, cost-effective pharmacotherapy, patient education, and collaboration with clinic physicians and pharmacists.

You will work in a hybrid model, performing phone-based and in-person assessments, while supporting evidence-based care and quality improvement initiatives across clinics.

Qualifications

• Must hold a Doctor of Pharmacy (PharmD) and be licensed in the state of employment.
• 3+ years' experience as a clinical pharmacist; PGY1/PGY2 residency preferred.
• Strong verbal communication skills and deep pharmacology knowledge; FDA guidelines knowledge.

Responsibilities

• Provide diabetes and obesity medication therapy management including reconciliation and adherence counseling.
• Collaborate with interdisciplinary care team to develop individualized care plans.
• Monitor regimens for therapeutic goals, adverse effects, and interactions.
