Job summary

Shields Health Solutions in Louisiana seeks an Ambulatory Clinical Pharmacist to advance diabetes and obesity pharmacotherapy through medication therapy management, reconciliation, and patient education.

The role is remote with hybrid on-site collaboration, requiring a PharmD and 3+ years of clinical pharmacy experience; PGY1/PGY2 residency is preferred. You will work with physicians, specialists, and clinic pharmacists to optimize outcomes and safety.

Qualifications

• PharmD required and licensed pharmacist in state of employment.
• At least 3 years of clinical pharmacist experience; PGY1/PGY2 residency preferred.
• Strong knowledge of pharmacology, FDA health regulations, and diabetes care.

Responsibilities

• Provide medication therapy management for diabetes and obesity, including reconciliation and adherence counseling.
• Collaborate with interdisciplinary care team to develop individualized care plans aligned with guidelines.
• Monitor and evaluate medication regimens for therapeutic goals, adverse effects and interactions.
• Identify opportunities for drug therapy optimization and cost savings initiatives.
• Participate in quality improvement initiatives and clinical outcomes studies.
