UMass Memorial Health is seeking a licensed practical nurse for remote telehealth in the United States. The role will support ambulatory and primary care patients with medication education, triage, and care coordination.

Responsibilities include nurse visits focused on medication administration education, documenting encounters in the EMR, and collaborating with clinical teams. A current LPN license and at least 2 years of clinical experience are required.

Qualifications

• Active LPN license in good standing.
• Minimum 2 years of clinical experience (ambulatory, primary care, or telehealth preferred).
• Strong communication and patient education skills.
• Proficiency with telehealth technologies and remote patient communication.

Responsabilités

• Conduct telephonic and video-based patient triage to assess symptoms and clinical concerns.
• Perform nurse visits focused on medication administration education and lifestyle assessments.
• Provide comprehensive patient education on medications and program requirements.
• Document all clinical encounters accurately in the EMR and ensure HIPAA compliance.
• Coordinate care and communicate patient updates with clinical teams and payers.
