Job summary

CareSource in Massachusetts is seeking an LPN to support clinic-based patient flow, collect medical history, perform phlebotomy and immunizations, and assist in developing and implementing patient care plans. The role involves measuring vital signs, administering medications, and collaborating with the care team to ensure quality outcomes.

The ideal candidate will hold an LPN license in MA with CPR certification and at least 2 years of clinical experience, including telehealth familiarity.

Qualifications

• Minimum of 2 years clinical experience.
• Willing to learn and utilize telehealth technologies (video, chat, etc.), when appropriate, for a variety of clinical care and care management services
• Willing to adapt to clinic team workflows and ensure patient safety and compliance with care protocols

Responsibilities

• Assist patients before and after examination, including dressing, undressing, positioning and transfers.
• Take and document patient history, including PHQ9 screenings.
• Perform phlebotomy, immunizations, specimen collection, and processing for labs.
• Assess health needs under supervision of the nurse.
• Establish care plans with physician, nurse and patient/family.
• Monitor patient status and document changes; respond to adverse reactions.
• Administer prescribed medications and record on charts.
• Measure and record vital signs.
• Provide basic patient care and treatments.
• Collaborate with care team to plan and modify interventions.
• Manage medical supplies and inventory; restock as needed.
• Coordinate scheduling and administrative tasks, including preauthorizations.

Education

High School Diploma, GED or equivalent experience

LPN License to practice in Massachusetts

Current CPR Certification
