Shields Health Solutions is hiring an Ambulatory Clinical Pharmacy Specialist to elevate patient care through remote collaboration with care teams at a nationally recognized Academic Medical Center. You will conduct virtual patient assessments, monitor treatment plans, and resolve medication problems under a CPA.

Qualifications

• PharmD required.
• WA pharmacist license required.
• PGY2 Ambulatory Care Residency or PGY1 + 1 year ambulatory CPA or 3+ years ambulatory practice.

Responsibilities

• Provide comprehensive medication management and reconciliation.
• Collaborate with physicians and care teams under a CPA.
• Monitor therapy effectiveness and safety; optimize treatment.
• Support quality-improvement and REMS initiatives.
• Assist operations with complex prior authorizations and training.
