Shields Health Solutions is seeking an Ambulatory Clinical Pharmacy Specialist to join our remote team and collaborate with care teams under a CPA to optimize patient therapy. You will conduct virtual patient assessments, order and monitor evidence‑based treatment plans, and resolve medication‑related problems while advancing quality improvement efforts and providing clinical development opportunities.

Requirements include PharmD, Washington license, PGY2 Ambulatory Care or equivalent, and

Qualifications

• Hold a PharmD and be licensed in Washington.
• PGY2 Ambulatory Care Residency OR PGY1 with ambulatory practice under a CPA.
• At least 3 years of ambulatory practice with disease state management.
• Strong communication, organization and tech skills.
• Empathy, integrity and a team-first mindset.

Responsibilities

• Provide comprehensive medication management including reconciliation, dose optimization, adherence support and drug-level assessments.
• Partner with physicians and interdisciplinary teams to order care plans under a CPA and coordinate medications.
• Monitor therapy effectiveness and safety and intervene as needed.
• Support quality improvement and REMS initiatives.
• Assist operations with complex prior authorizations, workflow improvements, patient education and ongoing clinical development.
