Job summary

Shields Health Solutions is seeking an Ambulatory Clinical Pharmacy Specialist to elevate patient care at a nationally recognized Academic Medical Center. This remote, CPA-guided role collaborates with care teams to assess patients, order and monitor evidence-based treatment plans, and optimize therapy in a hybrid Centralia/Olympia/Lacey area setting.

The position requires PharmD, WA licensure (or appropriate) and experience in ambulatory care; salary is $150,000–$190,000 with annual bonus

Qualifications

• Hold a Doctor of Pharmacy (PharmD).
• Must be licensed pharmacist in Washington.
• Completion of a PGY2 Ambulatory Care Residency.
• Completion of a PGY1 Residency and at least 1 year of ambulatory practice under a CPA.
• At least 3 years of ambulatory practice with disease state management.
• Strong communication, organization, and tech skills.
• Empathy, integrity, and a team-first mindset.

Responsibilities

• Provide comprehensive medication management, including reconciliation, dose optimization, adherence support, and drug-level assessments aligned with evidence-based standards.
• Partner with physicians, specialists, and interdisciplinary teams to order individualized care plans under a CPA and promote seamless medication coordination.
• Monitor therapy effectiveness and safety, identify optimization and intervene as appropriate.
• Support quality-improvement and REMS initiatives.
• Provide recommendations to operations by supporting teammates as needed with complex prior authorizations, improving workflows, educating patients, and maintaining ongoing clinical development.
