Job summary

Ria Health is seeking a Clinical Intake/Triage Nurse to support members in reducing or eliminating alcohol use through telehealth care. You will conduct standardized intakes, develop individualized care plans, and collaborate with physicians, nurses, and coaches to coordinate care while documenting sessions accurately in EMR systems.

The role is remote nationwide, with a focus on timely engagement, patient education, and adherence to evidence-based practices in addiction treatment.

Qualifications

• Certified Alcohol and Drug Counselor credentials required (AADC/ADC/CADC, or equivalent).
• 3–5 years experience in AUD or addiction treatment.
• Experience with EMR systems and telehealth tools.
• Familiarity with HIPAA/privacy and remote care workflows.

Responsibilities

• Conduct standardized intake assessments and form individualized care plans.
• Collaborate with medical and treatment teams to update care plans.
• Provide coaching via video/phone and app-based communication.
• Maintain caseload, monitor outcomes, and coordinate care.
• Document sessions promptly in EMR and billing systems.
• Support program development with clinical tools and teamwork.
