Job summary

Sailor Health is hiring a full-time, US-based remote Care Coordination Case Manager. You will own continuity of care for patients needing higher levels of care beyond virtual outpatient therapy, including high acuity and complex medical or social needs.

Responsibilities include identifying appropriate patients, assessing needs, planning transitions of care, and coordinating referrals and appointments. A Bachelor’s in a related field is required; Medicare experience is preferred.

Qualifications

• Bachelor’s degree in social work, nursing, psychology, public health, or related field required; master's degree a plus.
• Experience in case management, care coordination, discharge planning, utilization management, or referral coordination in a behavioral health or healthcare setting.
• Familiarity with behavioral health levels of care and how to determine fit.
• Experience with older adults, Medicare, or Medicare Advantage preferred.

Responsibilities

• Identify patients who are too high-acuity or out of scope for Sailor's model.
• Assess needs, insurance coverage, and barriers to care via phone or video.
• Plan individualized transitions of care to appropriate levels of care.
• Connect by placing referrals, scheduling first appointments, and confirming arrival.
