Job summary
Sentara Norfolk General Hospital is seeking a full-time Advanced Practice Provider to join the Case Management Pharmacy Program in Virginia. Work from home or at the hospital with a remote option.
The APP will perform health risk assessments, rewrite prescriptions, and coordinate care for about 150 patients monthly. Requirements include at least 1 year of APP experience in Family Medicine; Rheumatology or Infectious Disease experience is preferred.
