Sentara Health Plans in Norfolk, VA is seeking a full-time Advanced Practice Provider to join the Case Management Pharmacy Program. The role can be performed from home or at Sentara Norfolk General Hospital, with a Monday–Friday schedule from 8:30 AM to 5:00 PM, no weekends.

You will perform health risk assessments, rewrite prescriptions, and review about 150 patients per month, with a focus on Medicaid patients.

Qualifications

• 1 year experience as APP in Family Medicine.
• Rheumatology or Infectious Disease experience preferred.
