Job summary

Sentara Norfolk General Hospital in Norfolk, VA is seeking a full-time Advanced Practice Provider (APP) to join the Case Management Pharmacy Program. Flexible work from home or on-site at the hospital with a 8:30AM–5:00PM schedule, weekdays only, no weekends or holidays.

Primary location is Sentara Norfolk General Hospital. 1 year of APP experience in Family Medicine required; Rheumatology or Infectious Disease experience is a plus.

Qualifications

• 1 year experience as APP in Family Medicine.
• Experience in Rheumatology or Infectious Disease preferred.
• Proficient in conducting health risk assessments and rewriting prescriptions.

Responsibilities

• Perform health risk assessments.
• Rewrite prescriptions as needed.
• Review 150 patients per month.
• Collaborate with case management and pharmacy teams.
