Job summary

KBR is seeking a Special Operations Nurse Case Manager / Case Manager to provide professional nursing care and case management for DoD beneficiaries. The role includes analyzing care needs, coordinating services, and ensuring compliant, cost-effective patient care at Fort Bragg, NC.

Candidates must hold an RN (or equivalent) license, have 2+ years of nursing or case management experience, and pursue case management certification if not already certified. U.S.

Qualifications

• U.S. citizenship required.
• Active RN license or Licensed Clinical Social Worker in US territories.
• Two+ years nursing or case management experience.
• Case management certification within first year desired.

Responsibilities

• Collect, organize, record, and communicate data for health assessments to plan care.
• Assess patients via telephone using established protocols.
• Set priorities for patient care; monitor and evaluate progress.
• Oversee discharge-planning activities for timely care sequencing.
• Assist health care finders in referrals to providers or facilities.
• Negotiate and administer product/service agreements to meet care goals.
• Apply utilization review criteria; identify quality/risk issues.
• Follow VA/DOD Clinical Practice Guidelines for standardized care.
