Qualifications

• Current licensure; active and unrestricted RN or LCSW in a U.S. state or territory.
• Minimum two years nursing or case management experience; relevant certification within first year.
• Ability to communicate effectively with peers, providers, and stakeholders.

Responsibilities

• Collect, organize, and communicate health data to develop timely care plans for case managed patients.
• Assess patients by telephone using established protocols to provide cost-effective care.
• Oversee discharge planning to ensure timely, coordinated care transitions.
• Assist health care finders in identifying patient needs for referrals and services.
• Negotiate and administer care agreements to meet goals and compliance.
