Job summary

Lifepoint Rehabilitation in New Jersey is seeking a Registered Nurse (RN) to join our rehabilitation team. You will perform patient assessments, implement nursing interventions, and document medical necessity while contributing to an interdisciplinary care plan.

The role requires an active New Jersey RN license, BLS certification, and 6 months of acute care or med/surg experience. ACLS/CRRN are preferred but not required; schedule is 3x12, days, with rotating weekends.

Qualifications

• Applicants should have a current state RN license.
• Graduates from an accredited nursing program (ADN, BSN, or Diploma).
• Basic Life Support (BLS) certification is required.
• ACLS preferred; CRRN preferred but not required.
• Minimum six (6) months of acute care or med/surg experience preferred.
• Strong communication, critical thinking, teamwork, and rehabilitation-focused mindset.

Responsibilities

• Accurately performs patient assessments and identifies patient needs.
• Identifies and initiates appropriate nursing interventions.
• Provides care appropriate to condition and age of the patient.
• Performs timely and appropriate documentation relating to medical necessity in the medical record.
• Responsible for completion and revision of the Interdisciplinary Care Plan for each patient.
• Performs timely and accurate QI assessments.
