Job summary

Health Wealth Safe, Inc. is seeking an LPN/LVN to provide direct nursing care to adult rehabilitation patients with stroke, brain and spinal cord injuries, and other serious illnesses.

You will monitor vitals, administer medications, assist with activities of daily living, document care, and coordinate with RNs, therapists, and physicians to ensure safe, effective rehabilitation.

Commitment to safety and ongoing learning in a multidisciplinary setting is expected.

Qualifications

• Current LPN/LVN licensure.
• Strong medication administration, vital-sign monitoring, and clinical observation skills.
• Ability to support ADLs and work effectively in a multidisciplinary team.
• Accurate documentation and effective communication with the care team.
• Commitment to safety, rehabilitation principles, and continuous learning.
