Job summary

Preferred Home Health Care & Nursing Services is seeking an LPN/LVN to provide direct nursing care for adult rehabilitation patients with stroke, brain and spinal cord injuries, and complex orthopedic and neurological conditions.

You will administer medications, monitor vital signs, document care, and collaborate with RNs, therapists, and physicians to coordinate patient care. The role emphasizes safety, ongoing training, and a patient-centered approach within a multidisciplinary rehab team.
